Student Health Services听creates an听electronic听medical record (EMR) for every student when they submit their University entrance health records and when they are seen at Health Services. This record includes听entrance health documents, health-related forms, and treatment notes.听
Your Records are Private
Your confidentiality and privacy are protected in the delivery of听our services. Your medical records are maintained听on a secured server, and are viewed only by our staff in the course of service. We cannot release your private health information to any individual without your signed authorization. Exceptions to this authorization include medical emergencies, utilization review, quality assurance actions, court order or subpoena. If you would like to share your records with another party, you must submit a听Consent to Release Medical Information听for each visit or circumstance.
On your first health services visit听you will be asked if听you read and signed our Privacy Practices policy statement which is placed in your chart.听A copy of the statement is available to you whenever you ask or can be read.
Your records are also protected by the听(HIPAA) regulations and听听(FERPA).
Within our electronic medical record system you will be sent auto-generated emails reminding you of your next appointment and/or notifying you when a Health Services Staff member has sent you a secure message via the听.
We request that you reconsider sharing your 91成人导航Net username and passwords with those you trust. Should you decide to share your log-in information, you will unintentionally give that person access to view your health record information online. 听Items such as appointment history, messaging communication from Health Services Staff, lab results, immunization history and account balances will be available to any person who is able to log into your 91成人导航Net account. Any individual who has access to your email account will see messages sent to you reminding you of appointments and notifying you of pending messages on MyHealth@91成人导航. Therefore, we strongly recommend that you change your password of you have shared it to protect your privacy.

If you would like to opt-out of either of these two program features, please email the director of student health services Dayna Cerruti-Barbero @ dcerrutibarbero@pacific.edu. 听In your email, please indicate whether you would like to opt out of appointment reminders only, secure messages only, or both features. You do not need to send an email if you would like to have access to these two features. You may opt out of either or both of these features at any time. These features are turned on automatically upon enrollment at 91成人导航.
Access to Your Records
You are entitled to view and retain copies of anything in your medical record. To obtain copies of your records, you must present a photo ID and you must sign a听medical release听of information. Depending on the number of items you request, there may be a nominal charge for copies. Medical record requests must be made in advance and may require up to one business week to process.

If you would like more information about our privacy practices or have questions or concerns, please contact us. If you are concerned that we may have violated your privacy rights or you disagree with a decision we made regarding your protected health information, such as access, amendment, restriction or any other right, you may reach to us via email at studenthealthservices@pacific.edu. You also have the right to to submit a complaint to HHS (Health and Human Services). We will provide you with the address to file your complaint with HHS upon request.听
